About the Role

The professional nurse uses the nursing process to deliver patient- and family-centered care, valuing the uniqueness of each individual and addressing the physical, psychological, emotional, and social needs of the diverse patient population served by Jefferson Health.

Develops individualized nursing plans of care in collaboration with patients and their designated care partners, ensuring alignment with the patients' cultural beliefs and values.

Responsibilities

• Interacts with co-workers, patients, their families, designated care partners, and other staff consistent with Jefferson’s values.
• Integrates Evidence-Based Practice, clinical expertise, and patient/family preferences and cultural beliefs to deliver optimal healthcare.
• Partners with patients and their designated care partners to identify learning needs and provides education in accessible, affirming ways.
• Minimizes risk of harm to patients and others by adhering to established nursing practices, protocols, and individual performance standards.
• Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decision-making.
• Monitors outcomes of care processes using data and implements improvement methods to enhance quality and safety.
• Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making.
• Provides safe, compassionate, and coordinated care based on respect for patients' preferences, priorities, needs, and cultural beliefs.

Requirements

• Graduate of an accredited school of nursing (Specialized Diploma required).
• Active RN license in the state of practice (Pennsylvania).
• Minimum of 3 years of acute care RN experience.
• BLS certification (American Heart Association).

Preferred Qualifications

• Bachelor’s Degree in Nursing (BSN) or completion of BSN/advanced degree within 3 years of hire strongly preferred.
• ICU RN experience strongly preferred.

• Discharge Planning: Ability to partner with patients and families to develop discharge plans and arrange follow-up care.
• Assessment and Diagnosis: Knowledge of tools and techniques for patient assessment, nursing diagnosis, and clinical judgment.
• Health Information Documentation: Ability to record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, and test results accurately.
