About the Role
The RN, Nights and Weekends provides after-hours field visits to patients, families, and community partners. Establishes and maintains positive relationships with team members, patients/families, and referral resources.
Responsibilities
- Makes after-hours visits to patients in their home, assisted living facility, nursing home, or hospital within the assigned territory.
- Provides compassionate customer service to patients and families.
- Assesses patient/family and identifies immediate needs and appropriate nursing interventions by phone and/or during visits.
- Provides and manages direct care to patients and families as part of the Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical, and biological components.
- Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.
- Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care.
- Educates patients, families, caregivers, and other health professionals about disease processes, palliative interventions, caregiving, the dying process, and safety practices.
- Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals.
- Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support.
- Completes clinical admissions for all Company services, including:
- Explains services to potential patients/families and addresses questions or concerns.
- Identifies admission objections or barriers and provides responses.
- Educates patient/family on admission requirements and addresses immediate needs.
- Completes informed consents and initial assessments according to regulations and Company policy.
- Provides information to physicians and initiates Plan of Care to address patient’s immediate needs.
- Initiates skilled nursing interventions to prevent complications, alleviate symptoms, and maximize comfort.
- Obtains physician orders and completes documentation per Company policy.
- Acts as the Company representative at assigned facilities while facilitating referrals to all service lines; works closely with referring hospitals, physicians, facilities, patients, families, and the general public.
- Communicates frequently with Admissions to ensure timely follow-up and prepares for next assignments.
- Provides all necessary clinical communication timely using SBAR.
- Discusses potential needs with after-hours staff.
- Obtains consents for services and arranges safe discharge efficiently for referred and eligible patients while meeting productivity expectations.
- Obtains medical record information for referrals.
- Develops strong relationships with case managers, physicians, and other facility staff.
- Works with Professional Relations Representatives (PRRs) to provide education on all available services.
- Provides care for any General Inpatient (GIP) patients in a hospital, including daily visits, professional management, documentation, and efficient discharge.
- During emergencies (e.g., hurricanes), may be required to report to a designated location to ensure continuity of services, including staying overnight if necessary.
- Performs other duties as assigned.
Requirements
- Current license as RN in the state where the employee will be working.
- Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system.
- Ability to effectively communicate in English (verbal and written); bilingual in Spanish preferred.
- Valid driver’s license and automobile insurance per Company policy.
- Reliable transportation to meet patients’ needs.
- Excellent critical decision-making and problem-solving skills.

- Ability to use equipment with visual and auditory mechanisms.
- Excellent time management skills with the ability to prioritize demands to meet patient service standards and deadlines.

- Strong patient service skills, including information seeking, probing, accurate knowledge transfer, listening, understanding, and responding.
- Able to navigate in a fast-paced environment utilizing interpersonal skills during patient visits.
- Ability to work in a cooperative team environment.
- Able to exhibit a sense of urgency in daily work activities.
- Skilled in computer operations; proficiency in MS Office software applications, online research, and proven data entry and navigation skills.
- Available to work shifts to accommodate extended hours of operation as scheduled.
- Ability to perform sedentary work with frequent interruptions.
- Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
- Ability to perform the essential functions and physical requirements (including lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodations.
- Active BLS for healthcare professionals from the American Heart Association or Red Cross.
- Consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as ongoing compliance with the Drug-Free Workplace Policy.
- All employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment is made, as well as ongoing eligibility.

Pay
Pay Range: $67,976.53 - $106,213.33
