About the role

This role involves providing comprehensive home visiting services to pregnant and parenting women, along with their children, within a community setting. As an RN Home Visitor, you will conduct home visits, collaborate with community agencies, and ensure compliance with regulatory standards, policies, and protocols. This is a part-time, onsite position focused on delivering quality home healthcare and case management services in the Mahoning Valley area.

Responsibilities

• Conduct home visits for pregnant and parenting women and their children.
• Collaborate with community agencies.
• Ensure compliance with regulatory standards, policies, and protocols.
• Deliver case management.
• Support physical, psychological, social, educational, and safety needs of patients across all ages.
• Adhere to research protocols of the Nurse-Family Partnership program.

Qualifications

• Valid RN license.
• BLS certification.
• BSN (obtained within the last 5 years).
• At least two years of recent pediatric or related experience with children from birth to age five.

Skills

• Strong communication, especially with adolescents.
• Valid driver’s license with insured transportation.
