Nurse - Family Partnership Program RN

About the role

The RN Home Visitor plays a key role in delivering case management and support services within the Nurse Family Partnership program, focusing on pregnant and parenting women and their children.

Responsibilities

• Conduct home visits
• Collaborate with community agencies
• Ensure compliance with all regulatory standards, policies, and protocols
• Maintain high standards in clinical practice, professional development, teamwork, and research
• Provide comprehensive physical, psychological, social, educational, and safety care tailored to patients' needs
• Perform other duties as assigned to support the program's objectives

Qualifications

• Valid RN license
• BSN within five years
• BLS certification
• At least two years of recent pediatric or related experience with children birth through five
• Experience in adolescent psychology, OB, home care, or community nursing is preferred

Skills

• Strong communication abilities, especially with adolescents
• Valid driver’s license with insured transportation
