RN - Nurse Family Partnership Program

About the role

RN Home Visitor in a Nurse Family Partnership program, responsible for providing case management and support services to pregnant and parenting women and their children.

Responsibilities

• Conduct home visits with clients, adhering to research protocols and program guidelines.
• Collaborate with community agencies to coordinate care.
• Ensure compliance with the Ohio Nurse Practice Act, regulatory standards, and organizational policies.
• Deliver comprehensive care addressing physical, psychological, social, educational, and safety needs across all ages.
• Participate in professional development, teamwork, and performance improvement initiatives.
• Perform other related duties as required.

Qualifications & Skills

• BSN required within 5 years; RN licensure and BLS certification mandatory.
• At least two years of recent pediatric or related experience with children birth through 5 years.
• Experience in adolescent psychology, OB, home care, or community nursing preferred.
• Strong communication skills, especially with adolescents.
• Valid driver’s license and insured vehicle required.
