About the role
We provide skilled nursing care to people recently discharged from the hospital, needing therapy & to those needing long-term care.
Responsibilities
- Care Needs & Planning: Assist to develop, implement and continually update individual care plans for wound management
- Assessments: Complete regular wound assessments and document changes
- Triage & Interventions: Assist to handle necessary changes for wound care, physicians' orders and collaboration with care providers & families
- Regulatory Standards: Ensure compliance with federal, state, county & company health/safety regulations for staff education requirements
- Quality Assurance: Audit documentation & care for quality control/compliance
- Family & Team Liaison: Communicates changes in wound presentation to families/providers, works collaboratively with the interdisciplinary team
- Team Resource: Provide ongoing training, orientation & staff in-service for clinical skills, safety protocols & best practice
- Observes competency
Requirements
- Prior Long Term Care Experience
Qualifications
- Licensed to work within the State of Connecticut as a RN Registered Nurse
- Able to pass background check, drug screen, professional references check & physical evaluation
- High level understanding of leadership, customer service and clinical competency
- Experience in wound care management and staff education
- Experience in clinical leadership
