RN Coordinator (Family Medicine)

Contract-to-hire opportunity in Houston, TX (77014).

About the Role

RN Care Coordinator in a Family Medicine setting, responsible for coordinating patient care plans across providers, departments, and services.

Schedule

Monday–Friday, 8:00 AM–5:00 PM.

Pay

$40–$45 per hour.

Responsibilities

• Coordinate patient care plans across providers, departments, and services.
• Perform clinical triage via phone, EMR messaging, or in person.
• Review labs, imaging, and clinical results and escalate appropriately.
• Prepare patients for procedures and ensure pre-op requirements are met.
• Educate patients on treatment plans, medications, and follow-up care.
• Manage referrals and authorizations, especially in outpatient settings.
• Document all clinical interactions in the EMR accurately and timely.
• Serve as primary point of contact between patient, family, and care team.
• Monitor patient progress and intervene when care gaps appear.
• Support quality-improvement initiatives within the department.

Requirements

• RN License.
• BLS Certification.
• Experience in Family Medicine.
• Strong patient-education and care-coordination skills.
• EMR proficiency (Epic).
• Experience working in multidisciplinary teams.
• Ability to manage high patient volumes and complex cases.
• Care coordination or case management experience (1+ year preferred).

Bachelor's Degree in Nursing preferred.
