RN Clinical Supervisor
Essential Functions And Skills:
- Receives case referrals.
- Reviews available patient information related to case, including home visits, to determine hospice/home care needs.
- Sets up appropriate hospice personnel for cases as needed.
- Convenes with the Medical Director or Hospice Physician regarding any questions about an individual's eligibility for services.
- Reviews and evaluates each case through various means such as home visits, conferences, record review, and the services provided by clinicians; discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality hospice care services.
- Available at all times during operating hours to assist clinicians as appropriate.
- Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
- Aids clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plans of care.
- Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
- Conducts quarterly record reviews and communicates findings and recommendations to the Manager of Clinical Services and hospice personnel.
- Aids in the orientation of new employees.
- Affirms proper maintenance of clinical records in compliance with local, state, and federal laws.
- Aids Manager of Clinical Services and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs.
- Complies with accepted professional standards and principles.
- Affirms the health needs of the community and assists in the development of plan(s)/program(s) to meet needs.
- Promotes customer service orientation to all hospice personnel.
Additional Duties:
- Participates in on-call duties as defined by the on-call policy.
- Assumes responsibility for personal growth and development.
- Maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Actively participates in quality assessment and performance improvement teams and activities as assigned.
- Other duties as assigned.
Qualifications
- Must have a current Registered Nurse (RN) license in state(s) of practice.
- Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR).
- Maintain a graduate of an accredited school of nursing.
- Associate degree required.
- Proven ability to work within an interdisciplinary setting required.
- Ability to pass background, consumer report, and drug test.
- If Driving: Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.
- Preferred Bachelor’s degree preferred.
- Previous experience in hospice/homecare setting with 2 years management or supervisory experience strongly preferred.
Physical Requirements
- Pushing/Pulling and Lifting/carrying up to 30 pounds.
- Occasionally up to 50 pounds.
- Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.
