Key Responsibilities
- Provides oversight of all patient care services and personnel
- Covers patient and personnel assignments, coordinating patient care
- Coordinates referrals, assesses patient needs, and develops individualized patient plans of care
- Communicates regularly with support staff to assess needs and provide discussions
- Collaborates with the Director of Nursing / Clinical, Intake team, QA team, and branch staff for efficient case management workflow
- Assists with training and professional development of new and current staff
- Escalates priority cases and communicates with leadership on quality improvement and other issues
Qualifications
- Active RN license in good standing
- Prior experience in home health, intake, referral management, case coordination, or clinical triage preferred
- Strong understanding of home health services, documentation, and care coordination processes
- Excellent communication, organizational, and multitasking skills
- Strong attention to detail and ability to manage multiple cases simultaneously
- Experience with Kinnser (preferred)
- Knowledge of Medicare and other payer guidelines related to home health admissions
- Experience communicating with physicians, hospitals, rehab centers, and referral partners
- Ability to identify priority cases and escalate appropriately
- Comfortable using EMR systems and standard office software
