RN Clinical Supervisor

Essential Functions And Skills:

  • Receives case referrals.
  • Reviews available patient information related to case, including home visits, to determine hospice/home care needs.
  • Sets up appropriate hospice personnel to cases as needed.
  • Convenes with the Medical Director or Hospice Physician regarding any questions about an individual's eligibility for services.
  • Reviews and evaluates each case through various means such as home visits, conferences, record review, and the services provided by clinicians; discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality hospice care services.
  • Available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
  • Aids clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plans of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Conducts quarterly record reviews and communicates findings and recommendations to the Manager of Clinical Services and hospice personnel.
  • Aids in the orientation of new employees.
  • Aids Manager of Clinical Services and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs.
  • Complies with accepted professional standards and principles.
  • Assesses the health needs of the community and assists in the development of plan(s)/program(s) to meet needs.
  • Promotes customer service orientation to all hospice personnel.

Additional Duties:

  • Participate in on-call duties as defined by the on-call policy.
  • Attend interdisciplinary group meetings.
  • Assumes responsibility for personal growth and development.
  • Maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Actively participates in quality assessment and performance improvement teams and activities as assigned.
  • Other duties as assigned.

Qualifications:

  • Must have a current Registered Nurse (RN) license in state(s) of practice.
  • Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR).
  • Maintain a valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record if Driving.
  • Preferred Bachelor’s degree preferred.
  • Previous experience in hospice/homecare setting with 2 years management or supervisory experience strongly preferred.

Physical Requirements:

  • Pushing/Pulling and Lifting/Carrying up to 30 pounds. Occasionally up to 50 pounds.
  • Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.
