Clinical Leadership
- Conduct initial client assessments and develop individualized service plans
- Perform supervisory visits to ensure quality of care, caregiver competency, client satisfaction, and regulatory compliance
- Train, mentor, and coach caregivers on care plans, dementia care, safety, infection prevention, and best practices
- Maintain accurate documentation and support ongoing quality assurance and continuous improvement initiatives
Operations Leadership
- Partner with the office team to support scheduling, staffing, and continuity of care
- Help resolve client and caregiver concerns with professionalism, empathy, and urgency
- Improve workflows, strengthen operational processes, and identify opportunities to enhance efficiency and service quality
- Participate in the agency's rotating leadership on-call schedule
- Collaborate with leadership on strategic initiatives that support agency growth and operational excellence
Community & Business Development
- Build and strengthen relationships with hospitals, rehabilitation centers, assisted living communities, hospice providers, home health agencies, physicians, case managers, and other referral partners
- Represent Touching Hearts at Home through networking events, educational in-services, and community outreach
- Partner with leadership to grow referrals while reinforcing our reputation for exceptional service, responsiveness, and compassionate care
Qualifications
- Active North Carolina Registered Nurse license in good standing
- Passion for serving older adults and supporting families
- Strong leadership, communication, and relationship-building skills
- Organized, proactive, and comfortable making independent decisions
- Experience in home care, hospice, geriatrics, case management, or community-based nursing preferred
- Interest in operations, business development, quality improvement, and organizational leadership
- A desire to grow with a mission-driven organization and help shape its future
