Responsibilities
- Performs functions and patient care coordination consistent with the established plan of care and collaborates with team members to coordinate care/services to achieve optimal outcomes.
- Communicates to physicians/Non-Physician Practitioners (NPPs) and all disciplines involved in patient care regarding changes to the plan of care.
- Identifies challenges and potential barriers post-discharge to ensure appropriate resources and services are in place prior to discharge from an acute care or community setting.
- Assists in referring to appropriate community resources and services to optimize continuity of care.
- Drives optimal patient outcomes by working collaboratively with co-workers, client staff and other health care professionals to ensure continuity of care for the patient and family.
- Promotes cost-effective care by updating and communicating with staff and clinicians regarding strategic partner specific programs and processes in development.
- Provides continuing education and skills training for both office and clinical staff on current and new strategic partner specific programs and processes.
- Promotes a safe level of care for the patient through communication with agency staff and physician/NPP and other strategic partners (such as MCO, ACO organization staff); escalates needs for new or changed services to the supervisor.
Qualifications
- RN licensed in the state where patient care is delivered.
- Minimum of 5 years patient care experience.
- Working knowledge of Medicare Advantage health plans, ACO’s, IPA’s, ALF’s and LTC.
