Schedule: Full time 40 hours a week, 8:00-4:30, No Weekends or Holidays

General Requirements

  • Coordinates and manages patient care through the continuum of the patient’s hospital stay; including but not limited to evaluations, patient education and engagement, treatment, and scheduling discharge follow-up care, including post hospital Transitional Care Management (TCM) call within two business days after hospital discharge.
  • Meds reconciliation
  • In collaboration with the organization’s hospitalist, critical care cardiovascular teams, and other multidisciplinary departments, develops an evidence-based comprehensive plan of treatment for patients with HF through clinical pathways, algorithms, and protocols
  • Serves as a HF resource to all members of the healthcare team, patients and their families.
  • Ensures compliance with CMS standards, national guidelines, patient satisfaction, and cardiac registry for best practice
  • Works with physicians and staff to effectively manage referrals and process patients.
  • Educate and correspond with outlying facilities and community services regarding care of HF patients.

Education

  • Currently licensed by the State of Iowa as a registered nurse with 5 years acute care or advanced heart failure care
  • Bachelor’s degree required
  • Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire
  • Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers
