RN Clinical Case Manager (Pediatric Homecare)

H2 Homecare is an Atlanta-based pediatric homecare company looking for a dedicated and compassionate RN Case Manager to join our clinical team. This role is responsible for coordinating patient care, ensuring quality services, and serving as a liaison between patients, families, physicians, and caregivers. The RN Case Manager plays a vital role in helping patients receive exceptional care while maintaining compliance with clinical and regulatory standards. The ideal candidate is an organized, compassionate Registered Nurse with home care experience, excellent communication skills, and a passion for advocating for medically complex pediatric patients. This position is essential to ensuring continuity of care while supporting families and our clinical team. Responsibilities Conduct monthly in-home patient visits to assess patient needs and ensure quality of careComplete patient admissions, recertifications, care updates, and required clinical documentationEnsure compliance with company policies, state regulations, and clinical standardsServe as the primary clinical contact for assigned patients and familiesCollaborate with office staff to coordinate staffing and continuity of patient careProvide clinical guidance and support to field staff as neededParticipate in care conferences and communicate updates with the interdisciplinary team Qualifications Current and active Registered Nurse (RN) licenseMinimum of 2 years of home care experience requiredPreferred: 2 years of pediatric home care experiencePreferred: 2 years of GAPP experienceStrong assessment, communication, organizational, and problem-solving skillsAbility to work independently and effectively manage multiple patients and prioritiesCompassionate, patient-focused approach with excellent customer service skills Benefits Competitive salaryFlexible schedulingSupportive team environmentOpportunity to make a meaningful impact in the lives of children and their families
