RN Case Manager - Home Care

About the role

Provide head-to-toe assessments, care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

This position works in the field helping patients in their home setting. The office is located in Roseville, MN, and the general territory for this position covers the southwest section of the Twin Cities Metro area.

Responsibilities

• Perform assessments and procedures requiring knowledge, judgment, and nursing skill based on principles of psychological, biological, physical, and social sciences.
• Visit in-home clients to evaluate the quality of care being provided and review clinical notes.
• Participate in ongoing interdisciplinary assessment of the patient.
• Provide services ordered by the physician as indicated in the plan of care.
• Counsel and educate the patient/client and family in meeting healthcare-related needs.
• Complete timely and accurate clinical notes, including addressing the patient’s/client’s progress.
• Effectively communicate with other members of the interdisciplinary healthcare team to promote coordination of patient/client care and planning for discharge.

Requirements

• Graduate of an accredited school of nursing and a current license as a Registered Nurse in the State of MN.
• Able to travel within the geographic area serviced by the office.
• Reliable transportation, valid driver’s license, and proof of auto insurance are required.
• CPR Certification is required.
• Two (2) years of current nursing experience preferred.
• One (1) year of case management experience preferred.

Preferred Qualifications

• Community health experience.
• A wide range of Medical-Surgical experience.

Schedule

• Full-time | Days | Monday - Friday
• Holiday rotation (work approximately two holidays per year)
• Weekend rotation may apply dependent upon staffing levels
