RN Case Manager, Home Care

About the Role

One‑on‑one home care nursing with Saint Elizabeth Home Care, part of the nonprofit Saint Elizabeth Community (trusted care since 1882 and a 2026 PBN Best Place to Work in RI). The position includes conducting admissions, revisits, and discharges with real assessment and education time per client, supported by an interdisciplinary team, ample administrative assistance, iPad point‑of‑care devices, all necessary supplies, and mileage reimbursement.

Responsibilities

• Provide direct home care nursing to individual clients.
• Conduct patient admissions, revisits, and discharges.
• Perform comprehensive assessments and deliver client education.
• Collaborate with an interdisciplinary team to coordinate care.
• Utilize iPad point‑of‑care devices and supplied equipment for documentation and care delivery.
• Maintain accurate records and follow organizational protocols.

Requirements

• Active, unencumbered Rhode Island RN license.
• Current CPR certification.
• Minimum 1 year of nursing experience (home care experience preferred).
• Reliable transportation.
• Clean driver’s license and auto insurance.

Pay

Starting at $87,360+ per year.

Schedule

Salaried, Monday‑through‑Friday days with one weekend per month or less; full‑time (M‑F).
