About the role

The Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting. Collaborates with other inter-professional colleagues to plan, implement, and evaluate care. Documents resident history, symptoms, medications, and care provided in compliance with applicable standards/regulations. Utilizes technology to enhance quality resident care, collaborate with inter-professional team members, effectively delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment, and serve in leadership roles on department/organizational task forces and committees.

Demonstrates competency and practices within the full scope of nursing expertise/knowledge, utilizing appropriate age and population-specific standards as designated in the specific care area or unit. Must have effective communication skills to collaborate, negotiate, and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, residents, and families. Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations, including coordination of care, resident assessment, resident education, triage, and various other nursing interventions.

Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Responsibilities

  • Utilize the nursing process to provide individualized nursing care in the home setting.
  • Collaborate with inter-professional colleagues to plan, implement, and evaluate care.
  • Document resident history, symptoms, medications, and care provided in compliance with applicable standards/regulations.
  • Use technology to enhance quality resident care and collaborate with the inter-professional team.
  • Delegate to and supervise unlicensed personnel.
  • Assume overall nursing accountability for resident assignments.
  • Serve in leadership roles on department/organizational task forces and committees.
  • Demonstrate competency and practice within the full scope of nursing expertise/knowledge.
  • Communicate effectively to resolve conflicts and negotiate with peers, colleagues, residents, and families.
  • Analyze data and respond to changing situations with strong clinical reasoning and critical thinking skills.
  • Coordinate care, perform resident assessments, provide resident education, and conduct triage.
  • Adhere to the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules.

Requirements

  • Graduate from an accredited nursing program (e.g., CCNE, ACEN, or NLN CNEA).
  • Bachelor's degree in Nursing strongly preferred.
  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and maintains required department-specific competencies and certifications.
  • Basic Life Support (BLS) certification required.
  • Resident Assessment Instrument (RAI) process experience preferred but not required.
  • Depending on location, a valid driver's license may be required.

Schedule

  • Full time, weekday shifts with every 3rd week on-call rotation.
  • Weekly hours: 40.00

Pay

Salary range: $38.00 - $55.00 per hour
