RN / Clinical Care Leader - Full Time

Come join our nursing team at St. James. We are a family oriented work place where we care about our residents. Clinical Care Leader - Weekday shifts with every 3rd week on-call rotation. Candidate must be a Licensed RN.

Responsibilities

• Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting.
• Collaborate with other inter-professional colleagues to plan, implement and evaluate care.
• Document resident history, symptoms, medications and care provided in compliance with applicable standards/regulations.
• Utilize technology to enhance quality resident care and collaborate with inter-professional team members.
• Effectively delegate to and supervise unlicensed personnel.
• Assume overall nursing accountability for a resident assignment.
• Serve in leadership roles on department/organizational task forces and committees.
• Demonstrate competency and practice within the full scope of nursing expertise/knowledge.
• Utilize appropriate age and population specific standards as designated in the specific care area or unit.
• Collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, residents and families using effective communication skills.
• Analyze data and appropriately respond to changing situations using strong clinical reasoning and critical thinking skills.
• Coordinate care, conduct resident assessment, provide resident education, perform triage, and various other nursing interventions.
• Function within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
• Follow the Sanford Professional Nursing Practice which recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association.

Qualifications

• Bachelor's degree in Nursing strongly preferred.
• Graduate from an accredited nursing program, including but not limited to those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
• Minimum of two years' clinical experience preferred.
• Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
• Obtain and subsequently maintain required department specific competencies and certifications.
• Depending on location, a valid driver's license may be required.
• Basic Life Support (BLS) required.
• Resident Assessment Instrument (RAI) process experience preferred, but not required.
