Position Summary
The RN Clinic, Family Medicine delivers comprehensive nursing care to patients across the lifespan in an outpatient primary care setting. This role supports providers by performing patient assessments, administering treatments, coordinating care plans, and assisting with both routine and acute patient needs. Working collaboratively with physicians, advanced practice providers, and clinic staff, the RN helps ensure care is delivered safely, efficiently, and in alignment with clinical standards and Trinity Health policies. The position functions within a supportive, team-based primary care environment where communication, flexibility, and shared responsibility are essential. Through patient education, triage support, clinical follow-up, and accurate documentation, the RN Clinic, Family Medicine contributes to efficient clinic operations and positive patient outcomes across the Family Medicine service line.
Key Responsibilities
- Perform patient assessments, gather medical histories, obtain vital signs, and prepare patients for provider evaluation and preventive or acute-care visits.
- Administer immunizations, medications, and treatments according to clinical protocols, and assist with minor in-clinic procedures.
- Provide patient education on disease management, medications, preventive health, and follow-up care to support long-term wellness.
- Document all assessments, interventions, and communications accurately and timely within the electronic health record.
- Support clinic operations by maintaining exam room readiness, managing clinical supplies, and adhering to infection-prevention and safety standards.
- Collaborate with providers, care teams, and satellite clinics to coordinate appointments, diagnostic testing, and continuity of care.
