RN Case Manager - PRN

About the role

This PRN nursing position involves providing care management support across multiple healthcare settings, including hospitals and community-based environments.

Responsibilities

• Advocate for patients' health needs, focusing on responsible practices, self-care, and safety.
• Identify barriers to care and develop interdisciplinary plans with specific goals and interventions.
• Manage complex cases through assessment, resource deployment, and coordination to ensure seamless patient transitions between care levels.
• Utilize telehealth, virtual visits, and field-based approaches to deliver patient care effectively.
• Collaborate closely with inpatient teams for effective care handoffs.
• Engage patients and families to align care with their values, goals, and functional independence.
• Address gaps in care, utilize community resources, and uphold confidentiality and ethical standards.
• Document activities accurately, work independently within scope, and support organizational standards.

Qualifications

• Registered nurse with current licensure and a Bachelor's degree or higher in Nursing.
• Preferred CCM or RN-BC certification, or willingness to obtain within 36 months.
• Strong interpersonal, communication, and collaboration skills, proficient in technology and documentation.
• Knowledge of managed care, quality improvement, and clinical assessment, especially in cardiovascular and respiratory conditions.
• Ability to work in fast-paced, adaptable environments with critical thinking and problem-solving skills.
