Responsibilities

• Conduct initial client assessments and continuously refine care plans
• Provide hands-on nursing care and patient education
• Maintain open lines of communication with patients, families, and the healthcare team
• Manage patient care coordination for your caseload
• Participate in interdisciplinary team meetings to ensure comprehensive care planning

Requirements

• Licensed as a Registered Nurse (RN) in the state you work in
• Proficiency with Electronic Medical Records (EMR) software
• Excellent planning, organizing, and communication skills
• Ability to solve problems autonomously while maintaining high standards
• Valid Driver's License and reliable vehicle with current insurance
