RN Case Manager - PRN

About the role

The Care Manager, part of an interdisciplinary team, assesses, plans, advocates, and coordinates patient care from admission to discharge to ensure a safe transition post-hospitalization. They focus on maintaining patient safety, experience, and quality of care through effective transition planning and oversight of length of stay, utilizing performance metrics and best practices.

Responsibilities

• Assess and coordinate patient care efficiently and compassionately
• Advocate for patient needs and ensure safe, effective transitions
• Utilize performance metrics and best practices to enhance care quality

Requirements

• Graduate of an accredited School of Nursing (required)
• Current state license (required)
• BLS (CPR) certification at hire or within 90 days (required)

Qualifications

• 2-3 years of experience in an acute care hospital setting (preferred)
• Care Management or Utilization Management experience (preferred)
• Case Management certification (preferred)
