Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests compassionately.
- Partner with the Clinical Director to assess client eligibility and suitability for specialized hospice services.
- Provide direct care as an RN Case Manager, ensuring patient comfort and quality of life remain paramount.
- Deliver top-notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
- Empower patients and families through education, encouraging active participation in creating personalized care plans.
- Participate in call duties, demonstrating commitment to being there when it matters most.
Requirements
- Current unencumbered license as a Registered Nurse.
- Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting.
- Hospice experience is a plus.
- Computer proficiency, including experience with EMR systems.
- Current CPR certification (if required by state).
- Valid driver’s license, vehicle insurance, and access to dependable transportation.

Pay
$41.00 - $46.00 per hour. The pay range listed represents a general guideline; final compensation will be determined based on relevant experience, qualifications, and specific responsibilities.
