About the Role

Make a lasting difference as a Hospice Registered Nurse Case Manager (RNCM). Holisticare Hospice is seeking a per diem RN Case Manager to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.

Schedule

Per Diem, all shifts.

Territory

Ocean County.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests compassionately.
• Partner closely with the Clinical Director to assess eligibility and suitability of clients for hospice services.
• Provide direct nursing care, ensuring patient comfort and quality of life remain paramount.
• Deliver top‑notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
• Educate and advocate for patients and families, encouraging active participation in personalized care plans.
• Share call duties, being available when patients and families need support.

Qualifications

• Current, unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical‑surgical/acute care setting (hospice experience a plus).
• Computer proficient, including typing and EMR programs.
• Current CPR certification if required by state.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Pay

$43.00 – $50.00 per hour (average pay range). Final compensation will be determined based on experience, qualifications, and responsibilities, and will be discussed at the conclusion of the interview process.
