About the role
Join a growing team as a RN-Case Manager providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses.
Responsibilities
- Conducting weekly in-home nursing visits, performing physical assessments, ensuring home safety, managing medications, and supervising caregivers.
- Collaborate with regional staff to improve services, plan and evaluate care, and participate in client care conferences.
- Provide clinical support, ensure policy adherence, and assist with staffing, training, documentation review, and quality improvement activities.
- Support marketing efforts and ensure timely, quality care delivery.
Qualifications
- Graduate of an approved nursing program with a current, unrestricted RN license.
- Minimum two years of nursing experience, including one year in home care.
- CPR certification; BSN preferred.
- Supervisory or case management experience is a plus.
Skills
- Strong clinical assessment, communication, and teamwork abilities, along with adherence to policies and procedures.
