About the role
Join a growing team as a RN-Case Manager, providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses.
Responsibilities
- Conducting weekly in-home nursing visits
- Performing physical assessments
- Ensuring home safety
- Managing medications
- Overseeing caregivers
- Collaborating with team members to improve services, plan and evaluate care, and serve as a clinical resource in clients' homes
- Participating in staff hiring, training, documentation review, and quality improvement initiatives
- Supporting community outreach and ensuring compliance with agency policies and scope of practice
Qualifications
- Graduate of an approved nursing program, BSN preferred
- Current RN license, with a minimum of two years nursing experience, including one in home care
- CPR certification
- Supervisory or case management experience is a plus
