RN Case Manager - Paid Drive Time

About the role

We are seeking RN-Case Managers to provide in-home care for former Nuclear Weapons Workers with chronic or terminal illnesses caused by exposure to toxic substances.

Responsibilities

• Conducting weekly in-home nursing visits, including physical assessments, medication management, and home safety evaluations.
• Overseeing and supporting skilled and non-skilled caregivers.
• Collaborating with team members to improve service quality and ensure client needs are met.
• Participating in care planning, client care conferences, and clinical documentation review.
• Assisting with staffing, training, supervision, and performance improvement initiatives.
• Supporting marketing activities and maintaining compliance with policies and regulations.

Requirements

• Graduate of an approved nursing program; BSN preferred.
• Current, unrestricted RN license.
• Minimum two years nursing experience, including one year in home care.
• CPR certification; supervisory or case management experience preferred.
