About the role
Host Healthcare is seeking a travel nurse RN Case Manager, Acute Care Case Management for a 13-week assignment starting August 17, 2026.
Responsibilities
- Assess patient needs and develop care plans
- Coordinate care among various healthcare providers
- Manage patient transitions between facilities
- Monitor patient progress and adjust care plans as necessary
- Communicate with patients, families, and other healthcare professionals
Requirements
- Bachelor's degree in Nursing (BSN)
- Current licensure as a Registered Nurse (RN) in North Carolina
- Experience in acute care case management preferred
- Strong communication and organizational skills
Qualifications
- Passport required for international travel
- Valid driver's license and reliable transportation
