About the role
Care Team Solutions is seeking a travel nurse RN Case Manager, Acute Care Case Management for a travel nursing job in Burlington, North Carolina.
Responsibilities
- Manage patient care plans and coordinate care delivery
- Collaborate with interdisciplinary teams to ensure patient safety and quality of care
- Assess patient needs and develop appropriate care plans
- Communicate effectively with patients, families, and other healthcare providers
- Ensure compliance with regulatory standards and policies
- Document patient care activities and progress in medical records
Requirements
- Must have 2+ years of recent Case Manager experience in acute care settings
- Current licensure as a Registered Nurse in North Carolina
- Knowledge of case management principles and practices
- Excellent communication and interpersonal skills
- Ability to work independently and manage multiple tasks
Qualifications
- Graduation from an accredited school of nursing
- Current RN license in North Carolina
- Basic Life Support (BLS) certification
