RN Case Manager - Paid Drive Time

About the role

Join a dedicated healthcare team as an RN-Case Manager, providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses caused by occupational exposure.

Responsibilities

  • Conduct weekly in-home nursing visits, performing physical assessments, managing medications, ensuring home safety, and overseeing caregivers.
  • Develop and implement personalized care plans, coordinate care conferences, and evaluate patient progress.
  • Serve as a clinical resource, support staffing and scheduling, and ensure compliance with agency policies and scope of practice.
  • Participate in staff training, review clinical documentation, and contribute to quality improvement initiatives.
  • Assist with client and physician interactions, and support marketing activities as needed.

Additional Duties

  • Participate in hiring, training, and performance evaluations.
  • Monitor clinical documentation and participate in performance improvement activities.
  • Support staffing, equipment tracking, and community outreach efforts.

Qualifications

  • Graduate of a state-approved nursing program; BSN preferred.
  • Current, unrestricted RN license.
  • Minimum of two years nursing experience, including one year in home care or related field.
  • CPR certification; supervisory or case management experience preferred.

Skills

  • Strong clinical assessment and care planning abilities.
  • Excellent communication, collaboration, and organizational skills.
  • Ability to manage multiple priorities and work independently.

Pay

Competitive hourly rate of $47 plus shift differentials.
