RN Case Manager, Home Health - East Grand Forks - Part Time - Days

About the role

Integrates health care for clients from pre-admission to post discharge through coordination and sequencing of care. Enhances patient/resident flow and interdisciplinary communication, promotes early intervention, and evaluates outcomes based on integration among established clinical, financial, and utilization data. Functions in structured and unstructured health care settings, utilizing independent nursing judgment. Applies age-specific structured care methodologies for neonatal, pediatric, adolescent, adult, and/or geriatric patients/residents. Coordinates, facilitates, and negotiates with patients/residents, family members, and the health care team. Monitors, evaluates, and trends patient/resident responses using structured care methodologies. Fiscally responsible for controlling variances and duplication. Demonstrates independence, assertiveness, and critical thinking. Documents clearly, solves problems, and works with minimal direction. Incorporates teaching/learning principles and adapts teaching to age groups. Facilitates groups and implements projects as needed. Establishes priorities, demonstrates flexibility, and possesses strong organizational skills. Knowledgeable about reimbursement for services provided.

This position covers the East Grand Forks and surrounding areas and involves flexible staffing. Primarily day hours with scheduled weekends and occasional evenings. May be exposed to communicable or infectious diseases, hazardous materials, and environmental stressors.

Responsibilities

Requirements

• Bachelor's degree in Nursing strongly preferred.
• Graduate from an accredited nursing program (e.g., CCNE, ACEN, or NLN CNEA).
• Minimum of two years’ clinical experience preferred.
• Currently holds an unencumbered RN license with the State Board of Nursing where practicing and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
• Obtains and maintains required department-specific competencies and certifications.
• Depending on location, a valid driver’s license may be required.

Pay

Salary range: $36.50 - $51.00 per hour.
