RN Case Manager (Full-Time)

Role Overview

RN Case Managers assess patients, coordinate individualized care plans, and collaborate with healthcare teams to ensure appropriate, timely care. They advocate for patients, review diagnostic results, and manage discharge planning to facilitate safe transitions across the continuum of care.

Key Responsibilities

• Coordinate discharge planning to ensure patients are discharged to the least restrictive environment.
• Manage hospital activities related to case management and discharge processes.
• Ensure compliance with quality patient care standards and regulatory requirements.

Qualifications And Skills

• Minimum of two years of nursing experience.
• Graduate of an accredited nursing program; licensed/registered RN in NY State.
• Preferred: Bachelor's degree in Nursing and hospital case management experience.
• Current professional licensure; case management certification (preferred).
