RN Case Manager (Full-Time)

About the role

Assess patients, coordinate individualized care plans, and collaborate with multidisciplinary teams to ensure quality, timely care and appropriate discharge planning.

Responsibilities

• Coordinate discharge planning to facilitate patient progression through the care continuum and safe discharge to the least restrictive environment.
• Manage hospital activities related to case management and discharge planning.
• Ensure compliance with quality standards and regulatory requirements.

Requirements

• Minimum of two years of nursing experience.
• Active RN license in New York State.
• Preferred: Bachelor’s degree in Nursing.
• Preferred: Hospital case management or discharge planning experience.
• Maintain current nursing licensure; case management certification (e.g., CCMC) preferred.
