RN Case Manager
About the role
The RN Case Manager is responsible for carrying out many duties that directly affect customer service, quality care, and hospital reimbursement. This is a multi-focal role involving review of benefits for planned admissions, concurrent review of charts for medical acuity, coordination of admissions to supportive care, participation in discharge planning within the multidisciplinary care team, assistance in assignment of DRG's, and contacts with hospital departments and outside agencies.

Qualifications
- Associate's Degree in Nursing or experience commensurate to a Bachelor's degree (required)
- Bachelor's Degree (preferred)
- Advanced computer coursework (preferred)
- Five years acute care nursing experience (required)
- Prior related work experience in Patient Review or Case Management (preferred)
- Currently licensed by the Board of Licensing and Regulation in the State of Wisconsin as a Registered Nurse in good standing (required)
Skills
- Basic computer skills
- Beginning Microsoft Office Suite skills: able to open a document, change font, save a document, write a letter
Schedule
- Part-Time, .5 FTE
- Week 1: 24 hours
- Week 2: 16 hours
- Scheduled days: Variable between Monday - Friday
- Shift: 8:00 a.m. - 4:30 p.m.
- No weekends
