About the role
The RN Case Manager provides compassionate end-of-life hospice care in patients' homes or community settings. They assess, plan, implement, and evaluate care needs, supervise hospice aides, and coordinate services, ensuring patient dignity and comfort.
Responsibilities
- Manage patient and family caseload during on-call hours, collaborating with the triage team
- Report activities to clinical leadership and document care appropriately
- Assist with medication and supply delivery outside regular hours
- Participate in interdisciplinary team meetings during on-call periods
- Admit eligible patients in collaboration with the triage team, under physician orders
- Develop and update individualized care plans focusing on patient-centered goals and the 4Ms (What Matters, Medications, Mentation, Mobility)
- Support Age-Friendly Care practices
Qualifications & Skills
- Unrestricted RN license in the practice state
- Valid driver’s license and auto insurance
- Minimum of two years RN experience, with hospice/palliative care experience preferred
- Strong clinical judgment, excellent communication, compassion, and ability to handle sensitive situations
