RN Case Manager

About the role

The RN Case Manager is responsible for providing comprehensive care coordination services to patients in their homes. This includes managing patient care plans, ensuring timely medication administration, and coordinating with other healthcare providers.

Responsibilities

• Develop and implement individualized care plans for patients
• Monitor patient progress and adjust care plans as necessary

Requirements
