Lead with Heart

• Guide a multidisciplinary team of LPNs, Aides, Social Workers, and Chaplains in creating personalized care experiences
• Be the Advocate: Champion each patient's unique needs, preferences, and wishes while supporting their loved ones
• Create Meaningful Connections: Build relationships that matter, providing counseling and emotional support when it's needed most
• Develop and implement care plans that prioritize quality of life and comfort

Required Qualifications

• Active RN License
• Valid Driver's License
• 1+ years hospice experience

Preferred Qualifications

• BLS Certification
• Hospice and Palliative Medicine background
