RN Case Manager

As an RN Case Manager at Moments Hospice, you will play a critical role in making our patients' final days, weeks, and months as comfortable as possible. On-call support is available for our case managers, ensuring you always have the help you need.

Responsibilities

• Conduct initial client assessments and continuously refine care plans
• Provide hands-on nursing care and patient education
• Maintain open lines of communication with patients, families, and the healthcare team
• Manage patient care coordination for your caseload
• Participate in interdisciplinary team meetings to ensure comprehensive care planning

Requirements

• Licensed as a Registered Nurse / RN in the state you work in
• Proficiency with Electronic Medical Records (EMR) software
• Excellent planning, organizing, and communication skills
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards
• Valid Driver's License and reliable vehicle with current insurance
