RN Case Manager

About the role

As a Home Health Aide at Amedisys, you will provide compassionate care to patients in their homes. You will assist with daily living activities, medication management, and help patients maintain independence.

Responsibilities

• Assist patients with personal hygiene and grooming
• Help patients with meal preparation and nutrition
• Monitor patients' health and report changes to healthcare providers
• Manage medications and ensure patients take them as prescribed
• Support patients in maintaining their home environment
• Communicate regularly with patients, families, and healthcare providers

Requirements

• High school diploma or equivalent
• Valid driver's license
• Ability to pass a background check
• Pass a drug test

Qualifications

• Experience in a caregiving role preferred
• Strong communication skills
• Flexibility to work evenings, weekends, and holidays

Skills

• Excellent interpersonal skills
• Knowledge of basic first aid and CPR
• Ability to manage time effectively

Pay

Hourly wage based on experience and education.
