Our Care Managers are essential members of the healthcare team, positively impacting patient and organizational outcomes by coordinating care for medically complex patients in a tertiary-level acute hospital.

The Care Management Department consists of highly skilled Care Managers and Social Workers dedicated to providing excellent care for a diverse and complex patient population. This dynamic department continues to grow, offering opportunities for professional development.

Under general supervision, the RN Care Manager facilitates the patient's hospitalization from pre-admission through discharge, coordinating with physicians, nurses, social workers, and other healthcare providers to ensure medically appropriate and cost-effective care. The role involves applying clinical expertise and medical appropriateness criteria to resource utilization and discharge planning, as well as communicating with administration and the healthcare team to facilitate patient care.

Responsibilities

  • Facilitate patient hospitalization from pre-admission through discharge.
  • Coordinate with physicians, nurses, social workers, and other healthcare providers to expedite medically appropriate and cost-effective care.
  • Apply clinical expertise and medical appropriateness criteria to resource utilization and discharge planning.
  • Communicate with administration and the healthcare team to facilitate patient care.
  • Demonstrate understanding of patient flow and transitions to post-acute care and community-based services.

Requirements

  • Graduate from an accredited school of Nursing.
  • Registered Nurse (RN) license issued by the State of California.
  • BLS certification at time of hire, with commitment to obtain BART within six (6) months of hire.
  • Minimum of 3 years of acute care management experience.
  • Demonstrated utilization review and discharge planning experience.
  • Proficiency in Epic and CarePort.
  • Effective interpersonal communication, negotiation, leadership, and conflict management skills.
  • Ability to thrive in a fast-paced, dynamic environment with a commitment to professional growth and ongoing departmental performance improvement.
  • Proficiency in information systems, databases, and programs including MS Office (Word, Excel, Outlook, PowerPoint).
  • Fast and accurate typing, data entry skills, and proficiency with detailed written communication for electronic medical records.

Preferred Qualifications

  • BSN/MSN.
  • ACM and/or CCM certification.
  • Med/Surg, progressive care, or ED clinical RN experience.
  • UCSD RN experience or RN experience in another academic medical center.
  • Previous supervisory or management experience.
  • RN experience in a post-acute care facility or home health agency.
  • Proficiency with Epic, ECIN (Allscripts), and/or Utilization Review/InterQual.
  • Knowledge of local payers, coverage, reimbursement, and evidence-based clinical nursing practice.

Special Conditions

  • Ability to work various hours and locations based on business needs.
  • Must work at least four 8-hour shifts in a 4-week scheduled period, including a minimum of one weekend shift.
  • Employment is subject to a criminal background check and pre-employment physical.

Pay

Hourly pay scale: $91.52. Annual full pay range: $191,094 (prorated if the appointment percentage is less than 100%). Factors determining compensation include experience, skills, knowledge, abilities, education, licensure, certifications, and organizational needs.

Schedule

  • Per Diem appointment at 20% time.
  • 8-hour shifts, primarily Monday through Friday, with some weekends and holidays.
