About the role
We are seeking a Care Manager RN to perform case management functions for complex patients, coordinating care and resources across the continuum.
Responsibilities
- Planning, negotiating, and managing patient needs
- Communicating with medical staff, hospital personnel, and external resources to ensure appropriate utilization and adherence to clinical and financial standards
- Monitoring medical records for variances and intervening to optimize patient outcomes
Qualifications
- Associate's Degree in Nursing
- California RN license
- Minimum of 2 years of acute care experience in Medical-Surgical or Inpatient settings
- At least 1 year in care management or utilization review, or successful completion of relevant training programs
Preferred Qualifications
- Bachelor's Degree in Nursing or higher within 3 years
- Certified or Accredited Case Manager (CCM or ACM)
