About the role

The RN Care Manager facilitates patient hospitalization from pre-admission through discharge. They coordinate with physicians, nurses, social workers, and other healthcare providers to ensure medically appropriate and cost-effective care. The Care Manager applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. They communicate with administration and the healthcare team as needed to support patient care.

Responsibilities

  • Apply clinical expertise and medical appropriateness criteria to resource utilization and discharge planning
  • Communicate with administration and the healthcare team as needed

Requirements

  • Graduate from an accredited school of Nursing
  • Registered Nurse (RN) license issued by the State of California
  • BLS at time of hire with commitment to get BART within six (6) months of hire date
  • Minimum of 3 years prior inpatient RN care manager experience in an acute care hospital, or a minimum of 3 years of inpatient RN experience in the acute care hospital setting, or a minimum of 3 years combined inpatient RN care manager and inpatient RN experience in the acute care hospital setting, within the last 7 years
  • ADN RN must have a minimum of 3 years acute care manager experience
  • Demonstrated utilization review/discharge planning experience
  • Effective interpersonal communication and negotiation skills, leadership ability, and conflict management and resolution
  • Thrive and excel in a fast-paced, dynamic environment and committed to professional growth and ongoing departmental performance improvement
  • Demonstrated understanding of patient flow, and transitions to post-acute care and community based services
  • Proficiency in information systems, databases, and programs including MS Office: Word, Excel, Outlook, and PowerPoint, etc.
  • Must have fast and accurate typing, data entry skills and proficiency with detailed written communication appropriate for electronic medical records

Qualifications

  • BSN/MSN
  • ACM and/or CCM certificate
  • Three to five (3 - 5) years clinical RN experience or RN care management experience in an acute care setting
  • Med/Surg, progressive care, or ED clinical RN experience
  • UCSD RN experience or RN experience in another academic medical center
  • Previous supervisory/management experience
  • RN experience in a post-acute care facility or home health agency
  • Facile with Epic, ECIN (Allscripts) and/or Utilization Review / InterQual
  • Knowledge of local payers and coverage, reimbursement, and evidence-based clinical nursing practice

Skills

Strong critical thinking skills, creative discharge planning, and proficiency in information systems, databases, and MS Office tools.

Schedule
