About the role

The RN Care Manager II - IPCM collaborates with an interdisciplinary team to assess, plan, advocate, and coordinate patient care from admission to discharge, ensuring safe transitions post-hospitalization.

Responsibilities

Requirements

• Graduate of an accredited Nursing program
• Bachelor's degree in Nursing
• Current RN license
• BLS certification
• At least 1 year of acute care hospital experience

Preferred Qualifications

• Certification in Case Management
• Experience in care or utilization management
