Position Summary
RN Case Manager supporting colorectal surgery patients through pre-op and post-op care coordination.
Position Duties
- Coordinate pre-op and post-op patient care
- Schedule surgeries, imaging, and follow-up appointments
- Collaborate with providers and care teams
- Document care in Epic
Minimum Required Qualifications
- Active RN license (MN required)
- 5+ years of RN experience
- Previous RN CM Experience
- Epic experience required
Preferred Qualifications
- Surgical experience preferred
- Med-Surg background preferred
- Colorectal surgery experience a plus
Schedule
- Length of assignment: 12–20 Weeks
- Shift / Hours per week: Monday–Friday, Onsite 40 Hours per Week
Pay
Pay Rate: $1800.00 - $2700.00
