Job Responsibilities

As a Registered Nurse (RN) at Swan Lake Nursing & Rehabilitation, you will be responsible for:

• Providing and/or supervising direct patient care, including assessing, planning, implementing, and evaluating care based on age-specific components
• Coordinating care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction
• Administering medications and procedures per established policies and guidelines
• Collaborating with members of the interdisciplinary team to influence care outcomes

Requirements

• Bachelor's degree in Nursing (BACHELORS)
• Minimum 1 year of experience in a similar role (MINIMUM YEARS OF EXPERIENCE)
• Current New York State Registered Nurse (RN) licensure
