Responsibilities
- Assess and identify patient/caregiver problems
- Establish nursing patient/caregiver care goals
- Identify and implement interventions for all agency programs
- Document outcomes of care
- Work with physician to establish and implement medical plan of care for the patient
- Work closely with other members of the interdisciplinary group
- Communicate effectively both verbally and in writing with group members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations
- Establish and maintain therapeutic relationship with patient/caregiver to include providing emotional support and teaching
Requirements
- Education and Experience:
- Graduate of an accredited School of Nursing
- Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
- One year of experience in an acute, hospice, home health setting, or long-term care setting plus.
- CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
- Other:
- Valid state-issued driver's license required.
- Must carry automobile liability insurance at limits required by agency.
- Must have own transportation.
Qualifications
- Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
