Responsibilities

  • Identifies housing resources and develops relationships with community agencies.
  • Coincides community outreach with outside agencies, and visits locations where clients are discharged to.
  • Aids clients in completing facility consent forms upon admission.
  • Facilitates resident council, community meetings, and/or groups as instructed.
  • Transports clients to appointments to provide direct linkage to resources.

Requirements

  • A high school diploma or equivalent (e.g., GED, HiSET).
  • An associate degree in a social science field is preferred, combined with at least three (3) years of experience.
  • A bachelor’s degree in a social science field is preferred, combined with at least one (1) year of experience.
  • Experience must include providing support to individuals living with mental illness, working in a behavioral health setting, or performing non-professional healthcare or social services functions.

Qualifications

Pay

  • Starting Compensation: 18.88 - 26.00 USD Per Hour

Schedule

  • Monday-Friday, 8:30 AM-5:00PM
