About the role

The Residential Case Manager/QIDP supports individuals in a 24-hour living arrangement, authoring and implementing Individual Service Plans (ISPs), coordinating services, and ensuring compliance with legal standards.

Responsibilities

  • Develops and implements Individual Service Plans (ISPs)
  • Maintains confidentiality of information related to persons served
  • Facilitates enrollment and disenrollment procedures
  • Represents MRI in planning individualized programs and evaluating service effectiveness
  • Adheres to safety policies and procedures
  • Maintains payroll, financial, personnel, and information related to persons served in accordance with relevant laws and regulations
  • Communicates effectively and diplomatically in both oral and written form
  • Identifies and implements creative solutions to problems
  • Organizes and prioritizes work
  • Works flexible hours
  • Lifts and carries 50 pounds, assists in lifting and transferring non-ambulatory individuals, and maintains physical capabilities as outlined
  • Utilizes various communication tools and maintains necessary certifications
  • Counsels, advises, teaches, and resolves conflicts
  • Synthesizes and analyzes data/information

Requirements

  • Bachelor’s Degree in human services or related field
  • Qualified Intellectual Disability Professional (QIDP) certification
  • One year experience with the developmentally disabled and/or DD/MI
  • First-aid and CPR certification within 60 days of employment
  • Thorough understanding of developmental disabilities, mental illness, active treatment, principles of normalization, inclusion, least restrictive environment, choice, integration, self-empowerment, and functional skill development
  • Ability to make sound clinical judgments based on sound reasoning
  • Ability to communicate effectively and diplomatically in both oral and written form
  • Ability to identify and implement creative solutions to problems
  • Ability to organize and prioritize work
  • Ability to work flexible hours
  • Physical capabilities including lift and carry 50 pounds, full range of mobility, adequate vision, hearing, manual dexterity, and ability to communicate effectively

Qualifications

  • Must be able to maintain confidentiality of information related to persons served
  • Must be able to adhere to Safety Policies and Procedures of MRI
  • Must be able to complete MRI training requirements
  • Must have or be able to obtain first-aid and CPR certification within 60 days of employment and maintain certification throughout employment
  • Must have reliable transportation to conduct agency business and/or transport persons served as needed
  • Must maintain a valid Illinois Driver’s License and a good Motor Vehicle Record
  • Must maintain personal auto insurance with minimum coverage as specified
  • Must be able to meet the following physical capabilities: lift and carry 50 pounds, assist in lifting and transferring non-ambulatory individuals, and maintain physical capabilities as outlined
