Residential Case Manager

Full-time position coordinating services for persons with disabilities.

Responsibilities

• Set up and attend Person Centered Plan team meetings as required
• Frequently check with individuals in their residential and day settings to ensure their needs are being met
• Be the point of connection between individuals and their natural supports

Requirements

• Bachelor’s Degree in Human Services or Bachelor’s Degree and prior experience
• Must be able to pass a drug screening, background check, and DOT physical

Schedule

• Main hours: Monday–Friday, 8:00 AM – 4:00 PM
• Additional hours dependent on the needs of the organization

Pay

$46,342 – $51,022
