Residential Case Manager (MHP Required)

About the role

Community House Mental Health Agency provides a supportive family atmosphere in Seattle and King County for people with severe and persistent mental illness. The Case Manager, Mental Health Professional (MHP) delivers master’s-level clinical services within CHMHA’s licensed residential programs to meet Washington State residential staffing requirements for clinical oversight and direct therapeutic services. This position is not supervisory but functions as a clinical resource to support non-MHP case managers and residential staff through consultation, guidance on clinical interventions, and continuity of care within a multidisciplinary team model.

Responsibilities

• Provide direct individual mental health counseling and therapeutic interventions to residents in accordance with service plans and clinical need.
• Provide clinical consultation and guidance to residential staff and non-MHP case managers through collaboration and shared clinical planning to support resident care.
• Contribute to meeting King County residential licensing requirements by providing required clinical services and therapeutic support to residents, consistent with Mental Health Professional (MHP) standards.
• Serve as Primary Case Manager for assigned caseload, including advocating for and coordinating resident needs and services, ensuring timely completion of Service Plans, Crisis Plans, Continuing Stay Reviews, and other documentation.
• Support the clinical work of non-MHP case managers through collaboration, shared documentation standards, and consultation on treatment planning.
• Coordinate with internal and external departments to facilitate resident referrals, timely intake, and review of individualized plans.
• Collaborate with the care team to ensure residents access needed treatment, support, case management, and appointments, including transportation to external providers.
• Ensure all interactions represent the five principles of trauma-informed care: safety, choice, trustworthiness, collaboration, and empowerment.
• Facilitate, monitor, and support medications and medication windows to ensure the seven rights of medication use are followed.
• Actively participate in Shift Change meetings and complete associated documentation.
• Complete daily documentation in the electronic medical record on resident interactions, medications dispensed, and other required documentation.
• Practice good financial stewardship of agency and contract resources.
• Meet identified key performance indicators and metrics for the position.
• Participate in discharge planning and written summaries of treatment as needed.
• Assist with and support on-site cleaning, including skills training with residents to maintain personal and community spaces, laundry, and preparation for regular pest treatments.
• Provide immediate crisis intervention and resolution as needed.
• Maintain resident and staff confidentiality.
• Complete assigned work related to licensing responsibilities for Washington Department of Social and Health Services (DSHS) and Department of Health (DOH).
• Provide skills training and assistance to residents in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
• Prepare resident meals when the cook is not on duty and per menu.
• Learn and execute skills and techniques promoted in the State of Washington mandated training, Fundamentals of Caregiving.
• Coordinate care, including scheduling, attending, and/or arranging transportation for appointments and other case management needs.
• Maintain and document in resident charts, activity logs, meal charts, etc., following agency, county, state, and federal guidelines.
• Facilitate regular group and individual activities, encounters, and specialized resident events, such as outings and holiday gatherings.
• Assist in admission and discharge planning per supervisor direction.
• Participate in fire drills, resident orientation, chores, and resident room maintenance during move-in and move-out.
• Monitor and manage facility milieu for visitors, television time, and quiet times.
• Attend all scheduled meetings and supervisions.
• Stay awake on all shifts.
• Perform other duties as assigned.

Qualifications

Master’s-level clinical training and credentials as a Mental Health Professional (MHP) in the state of Washington.

Schedule

Monday-Friday, 8:00am-4:00pm (Full-Time, 40 hours/week).
